As has been often descried, the well-being and integration of refugees, asylum seekers and immigrants mutually influence each other. Therefore, OMEGA Health Care Center is carrying through programs which at the same time further integration on the level of individuals and help prevent or address psychological, social and medical problems. Women and minors are the main target groups of these programs. Aside form giving adolescents concrete support—e.g. in their schoolwork—most of our youth programs focus on active work toward integration. This (...) we understand as supporting real contact between individuals of different cultures. Currently, OMEGA is conducting outreach activities in refugee shelters and schools. In afternoon activities minors have the opportunity to join a music group, to participate in a school support program and to attend computer courses. One goal of our work it to include marginalized youth in all these groups and actively support their social integration in the mainstream society. Finally, the results of an evaluation of our music group and possible interventions for group leaders are discussed. (shrink)

Mainstream breast cancer social movements such as that of Susan G. Komen have called on all women to race or fight for the cure for breast cancer. They suggest that the fight can be won by buying and wearing pink ribbons, taking part in races and walks, donating money, and participating in research. For some patients with breast cancer, research participation may involve non-diagnostic tumor biopsies. While this clinical research is performed in the hope of gaining new knowledge through (...) the removal and study of breast cancer tissue, these procedures have been heavily criticized because they offer little potential benefit to the patient providing the tissue (Helft and Daugherty 2006; Kimmelman et al. 2012;... Helicopter cabin noise was simulated by combining a broadband signal (pink noise, or PN) with a triad of pure tones (PT) at 650,1900, and 5000 Hz. Each component was presented at four loudness levels (0,60,70, and 80 dB[A]), with all 16 combinations arranged in two unsystematic orders. Intelligibility was measured by means of sentences to be judged as true or false. A male speaker presented 10 sentences at each noise condition. One group of subjects heard the sentences at 50 (...) dB(A) and another group at 55 dB(A). The annoyance of each noise combination was rated by the same subjects on a 5-point scale. Both sources and their interaction were significantly related to reduced intelligibility and increased annoyance at both speech levels. In general, the PN component was more responsible for loss of intelligibility, and the PT component more responsible for annoyance. (shrink)

Classical temples in ancient Greece show two deterministic illusionistic principles of architecture, which govern their functional design: geometric proportionalism and a set of illusion-strengthening rules in the proportionalism's stochastic margin. Animal morphology, in its mechanistic-deductive revival, applies just one architectural principle, which is not always satisfactory. Whether a Greek Classical situation occurs in the architecture of living structure is to be investigated by extreme testing with deductive methods.Three deductive methods for explanation of living structure in animal morphology are proposed: the (...) parts, the compromise, and the transformation deduction. The methods are based upon the systems concept for an organism, the flow chart for a functionalistic picture, and the network chart for a structuralistic picture, whereas the optimal design serves as the architectural principle for living structure. These methods show clearly the high explanatory power of deductive methods in morphology, but they also make one open end most explicit: neutral issues do exist. (shrink)

Background: Only data of published study results are available to the scientific community for further use such as informing future research and synthesis of available evidence. If study results are reported selectively, reporting bias and distortion of summarised estimates of effect or harm of treatments can occur. The publication and citation of results of clinical research conducted in Germany was studied.Methods: The protocols of clinical research projects submitted to the research ethics committee of the University of Freiburg in 2000 were (...) analysed. Published full articles in several databases were searched and investigators contacted. Data on study and publication characteristics were extracted from protocols and corresponding publications.Results: 299 study protocols were included. The most frequent study design was randomised controlled trial , followed by uncontrolled studies , laboratory studies and non-randomised studies . 182 were multicentre studies including 97 international collaborations. 152 of 299 had commercial funding and 46 non-commercial funding. 109 of the 225 completed protocols corresponded to at least one full publication ; the publication rate was 48%. 168 of 210 identified publications were cited in articles indexed in the ISI Web of Science. The median was 11 citations per publication .Conclusions: Results of German clinical research projects conducted are largely underreported. Barriers to successful publication need to be identified and appropriate measures taken. Close monitoring of projects until publication and adequate support provided to investigators may help remedy the prevailing underreporting of research. (shrink)

Background: The magnitude of bullying and harassment among psychiatrists is reportedly high, yet no peer-review published studies addressing this issue could be found. Therefore, it was decided to conduct a pilot study to assess the degree of the problem, the types of bullying/harassment and to provide some insights into the situation.Methods and Principal Findings: Following multiple focus group meetings, a yes/no response type questionnaire was developed to assess the degree and type of bullying and harassment experienced by psychiatrists. Over a (...) 3-month period the questionnaire was administered to a random sample of 60 psychiatrists. 57 out of the 60 psychiatrists reported harassment and bullying. Frequencies of the following response variables are presented in descending order: rumours 40% ; defamation 20% ; passing remarks 20% ; false accusations 15% ; threats 13.3% ; verbal abuse 13.3% ; unjustified complaints 13.3% ; promotion blocked 13.3% ; humiliation 13% ; bad reference given 10% ; credentials questioned 8.3% ; physical attacks 5% ; termination 5% ; derogatory remarks 1.7% and 1.7% were subjected to personal work. As a result of being subjected to harassment, 66.7% of the psychiatrists did not take any action, whereas 33.3% confronted the person they believed responsible. Asked whether the bullying and harassment caused distress, 18.3% of the psychiatrists did not report any effect, 30% reported mild distress, 40% moderate distress and severe distress was reported by 11.7%.Conclusions: It was concluded that the magnitude of bullying and harassment among psychiatrists may be quite high, as evidenced by this pilot study. There is a need for extensive systematic studies on this subject and to establish strategies to prevent and address this issue at a national and regulatory level. (shrink)

The theoretical and practical problems of providing incentives for people's activity in society are becoming increasingly more urgent as the role of the human factor in the development of society grows. In light of modern historical experience, we can see the onesidedness of conceptions according to which the types and directions of activity are mechanically predetermined by conditions external to it, and we can see the necessity of understanding the laws of activity itself in all their complicated dialectical essence. These (...) problems have become particularly important at the present stage of development of Soviet society, when a greater active involvement of the masses and profound changes in social psychology are becoming acutely necessary. This paper describes the topological aspect of a logic-based, artificial intelligence approach to formalising the qualitative description of spatial properties and relations, and reasoning about those properties and relations. This approach, known as RCC theory, has been under development for several years at the University of Leeds. The main rationale for this project is that qualitative descriptions of spatial properties and relationships, and qualitative spatial reasoning, are of fundamental importance in human thinking about the world: even where quantitative spatial data (...) are most important, they must be attached to the components of a perceived spatial structure if we are to make use of them. RCC theory covers other qualitative aspects of spatial description and reasoning, but the topological properties and relations of spatially extended entities are fundamental to our work. The topological formalisms used by mathematicians are, in general, not well suited to the task of formalising the kinds of ‘common-sense’ or ‘everyday’ qualitative spatial description and reasoning which are our primary interest. Nevertheless, we must come to grips with the concepts of topology as practised by mathematicians if we are not to risk constantly ‘reinventing wheels’. (shrink)

The author, head of a teaching hospital surgical unit, argues that the medical curriculum must ensure that all students are exposed to a minimum of ethical discussion and decision-making. In describing his own approach he emphasises the need to show students that it is 'an intensely practical subject'. Moreover, he reminds them that moral dilemmas in medicine--perhaps a better term than medical ethics--are unavoidable in clinical practice. Professor Johnson emphasises the need for small group teaching and discussion of real cases, (...) preferably chosen and 'worked up' by individual students. The Athenian expedition led against Macedonia by Archestratos, son of Lykomedes, early in 432 was not diverted from its destination by the revolt of Poteidaia. Archestratos had received additional instructions to enforce the Poteidaia ultimatum if he could, but, this being already impossible, he continued with the real object of his mission, the attack on Perdikkas II of Macedon. The widespread revolt among the Chalkidians had deprived the Athenians of the bases for this attack on which they might have reckoned, (...) and Archestratos had at the outset to make good this loss by recapturing Therme, at the head of the gulf to which it gave its name. Therme, or Serme, had been a tributary member of the Delian League since 450/449:2 it need not necessarily be said that it lay within Macedonia, as Poppo and Bergk inferred from Thucydides 1. 61. 2, but it is at least likely that it lay on the boundary, as it was handed over to Perdikkas under the agreement of 431 .3 However, that the Athenians could include in their Empire a city within Macedonian territory is shown from the position of Berge , and is likely in other cases, e.g. Haison. From Therme, Archestratos moved on to Pydna, presumably by sea, and laid siege to it. Here he was joined by substantial reinforcements, while at the same time the situation in Chalkidike became increasingly embarrassing, so that a peace and a reinsurance alliance were seen by both sides to be ‘imperative’ . Dilemmas about resuscitation and life-prolonging treatment for severely compromised infants have become increasingly complex as skills in neonatal care have developed. Quality of life and resource issues necessarily influence management. Our Institute of Medical Ethics working party, on whose behalf this paper is written, recognises that the ultimate responsibility for the final decision rests with the doctor in clinical charge of the infant. However, we advocate a team approach to decision-making, emphasising the important role of parents and nurses in the (...) process. Assessing the relative burdens and benefits can be troubling, but doctors and parents need to retain a measure of discretion; legislation which would determine action in all cases is inappropriate. Caution should be exercised in involving committees in decision-making and, where they exist, their remit should remain to advise rather than to decide. Support for families who bear the consequences of their decisions is often inadequate, and facilitating access to such services is part of the wider responsibilities of the intensive care team. The authors believe that allowing death by withholding or withdrawing treatment is legitimate, where those closely involved in the care of the infant together deem the burdens to be unacceptable without compensating benefits for the infant.
